Maybe a person who knows he's insane and feels insane is actually more in touch with reality than a sane person. Maybe we are all batshit crazy. This is my "I'm tired and this is what my brain is telling me" post for the day so it may be a little unhinged but maybe it's only when tired that I see through the cracks. But then sanity is more than simply seeing things clearly. A lot of poets and artists do that. Maybe it's adjusting to the reality of the situation.

A lot of people who are normal do things that are utterly insane.

JMO but having a sticky work/personal/familial relationship with someone with bona fide serious mental illness or personality disorder will disabuse of you these notions.
