Unfortunately (to nobody's surprise) a media story doesn't contain enough details, or even both sides of the story. I cannot conceive of how the head would come off, even with a complete dystocia (stuck baby). It is also very surprising that there was dystocia at 28 weeks - usually the baby is so small that it comes out without much pushing. Which might be [speculation] why the OB did not want to go to c-section in an extremely early delivery.
Pushing the baby back in and going to c-section is not an unknown technique. Injuring the baby during a crash c-section is also not unknown. Taking the head off - that is a new one on me.
So, a couple of possibilities. Atlanto-occipital dislocation. The neck breaks at the first vertebra, and the head essentially pops off from a skeletal standpoint. The skin and muscles are still present, though. Non-survivable injury.
Second, brought up by one of the commentors, was an intrauterine fetal demise - the baby dies in the uterus, and the body eventually goes into labor to expel.
Third: this was an instrument associated delivery (forceps, vacuum) and that produced the injury.

If this is real there is nothing funny about this shit at all, Most of you are fathers. Imagine if this was your child?
Dead doctor right there and then. No hesitation, seize, break neck, crush trachea, drop him to floor and stomp the head untill my foot breaks. God help (he won't) anyone who tried to interfere.
I'd be out in a couple of years if I even went to prison. Temporary insanity through overpowering emotional shock and trauma.
Horrid. I hope it's bullshit.
